Reporting your Lost or Stolen Passport

 DS-64You need to report that your passport is lost or has been stolen. Submit this along with a DS-11 form to apply for a new passport.

Parental Consent for Applicants Under 16

Minors must appear in person to submit their application.

Both parents or legal guardians must authorize the issuance of the child’s passport.

Submit the following if:

  • Evidence of child’s relationship to the parents/guardian(s).
  • Both the original and photocopy of parent/guardian identification (front and back).
  • Evidence of sole parental authority

  • Notarized statement from both parents or legal guardians granted a third party permission to apply for the child.

 Consent to Issue Child Passport
(DS-3053)
One parent or guardian cannot go with the child to apply in person for the child’s passport. This form is to be submitted in addition to child’s passport application (DS-11). 
 Statement of Special Family Circumstances
(DS-5525)
One parent or guardian cannot locate the other parent or guardian. This form is to be submitted in addition to the child’s passport application (DS-11).

Parental Consent for Applicants Age 16-17

Minors must appear in person to submit their application.

Applicants 16-17 must show that at least one parent or guardian knows that they are applying for a passport. Parental awareness may be shown through one of the following methods:

  • A parent or legal guardian appears with the applicant.  The passport agent accepting the application will ask the parent or legal guardian to sign Form DS-11.  Bring a photocopy of that parent or guardian’s ID.
  • A signed statement consenting to issuance of a passport from at least one parent or legal guardian. The statement should be accompanied by a photocopy of that parent or guardian’s ID.
