REQUIRED DOCUMENTS

PROOF OF UNITED STATES CITIZENSHIP
One of the following:

original STATE-issued birth certificate with raised seal
certified copy of STATE-issued birth certificate with colored seal (note: CA and TX "abstract of birth" certificates are not acceptable)
birth records issued by a hospital are not acceptable
current or expired U.S. passport
original naturalization certificate

You may be required to submit the following in addition to proof of citizenship.

original name change documents
(marriage license, divorce decree, adoption papers)

IDENTITY VERIFICATION
One of the following:

current state-issued driver's license
current state-issued ID card
current military ID card

FEE PAYMENT
Applicants will need one personal check or money order for the passport fees, payable to Passport Services. This check or money order will be attached to the passport application. Credit cards and cash are not acceptable methods of payment for the passport fee.

Applicants can pay the execution/facility fee and photo fee (if applicable) to UC Regents with a personal check, money order, cash, credit card or debit card. The acceptance fee is set by the Department of State and is the same no matter where you apply for your passport.

The UCI Passport Application Acceptance Office will send your passport application to the Passport Agency via overnight mail at our expense.

PASSPORT PHOTOS
Two 2" x 2" identical recent passport photos.
Photo service available here for $15.

CHILD/YOUTH PASSPORTS

children under 16 years of age must apply in person with both parents present (for consent and proof of identity)
for children presenting a passport to prove citizenship, a birth certificate will also be required to verify the signing parent/s' relationship to the child
youths 16 or 17 years of age must apply in person with one parent present (for proof of identity)

Renewal by Mail

Adults may qualify for mail renewal and save the $25 execution fee. Requirements are printed on the DS-82 renewal form which you must mail in yourself. Visit our office for Renewal by Mail forms and passport photos.
